Musicians Injuries: OUCH, It Hurts When I Play (But Please Don’t Tell Me To Stop!)

I have a very personal connection to musician’s injuries. I had trained as a classical clarinet player and Psychology Degree was during my undergrad that I started to have all sorts of problems from playing too much and with Bluecross Blueshield posture. As a musician, it can be very difficult to explain to a physician or physiotherapist or even another chiropractor what the mechanics look like when you Commonwealth Banking playing your instrument. But when someone comes into my office and says that they play flute, or guitar, or tuba, or whatever, I know exactly what the physical component of playing their instrument involves. That is a very important first step.

Secondly, not only do you have to be able to have a good understanding of what playing that instrument involves, but you have to be able to see that person play. Even if someone tells me they play violin (I automatically think: “ok, so they will be leaning their head to the left and have right shoulder problems, etc…”), I am often shocked to see how over the years of playing they have contorted themselves into a little pretzel while they play!

So, on the first or second visit, all of my musicians bring in their instruments and I do a thorough playing analysis to see what it is that they’re doing right and wrong. It could be that their posture is contributing to their injury. Or maybe there’s something about the instrument that we could change; it might just need a minor adjustment in the thumb rest or a key positioning.

For instance, I have very small hands and found it difficult to reach some of the alternate fingering keys on my clarinet - so I had them sawed off and re-soldered on in a different direction so I could reach them.

Thirdly, it is important to recognize that there are some really common reasons for performance injuries. The most common ones are a change in repertoire, a change in the instrument (such as a new mouthpiece or something similar), a change in practice time or an upcoming recital. If we can pinpoint what it is that the performer has been doing differently lately to contribute to their injury, that helps immensely.

And lastly, it is so important to realize, especially for freelance artists, that you can’t just tell them to take a muscle relaxant, and take a few weeks off. If these people took a few weeks off, they wouldn’t have a roof over their head or food on the table. While it’s occasionally absolutely imperative that a break be taken, most of the time I take a holistic approach to treating performers and change and fix what we can, within the obvious limitations of current gigs and upcoming events.

3) What’s the most common injury that you see in your office?

In my office, there is a tie for the most common injury. The first is upper back/shoulder/neck pain - I lump these together because those terms can mean the same thing to a lot of people - often someone will come in and say that their shoulder hurts and point to the pain, but to me what they’re pointing to is actually their upper back or lower neck. This one is often a function of poor posture or poor practice ergonomics. If we can figure out how to improve the overall posture and ergonomic situation then China International Business tends to resolve quickly.

And the second most common injury is hand and arm pain. You would not believe how many people walk into my office with numb and tingly hands and fingers - which can be very scary if you’re the one to experience it - to find out that the problem isn’t actually their hands and fingers at all, but it’s a little further up the arm and can be quite easily treated once properly diagnosed. Or they come in with tennis elbow - but they have never held a tennis racket in their life! In my office, I call tennis and golfer’s elbow “musician’s elbow” because it is a repetitive strain injury. It is really, really common and surprisingly easy to treat.

4) What can musicians do to prevent injury?

First of all, don’t be a hero! There is just no reason to practice for hours on end without a break. Always remember to take a little break for every 30 minutes that you are playing. Secondly, don’t play through pain. The pain signal is there to tell you that you are doing something wrong. Playing through it is not going to get you anywhere - other than in more pain and in worse shape down the road. Thirdly, be aware of your ergonomics. If you sit to play, does your chair fit you properly? In rehearsal, do you have to strain at all to see both the stand and the conductor? Are your arms contorted oddly in order to be able to play properly? This is not good. And lastly, seek the help of a professional who can not only help you with the injuries that you are currently dealing with, but can help you avoid future injury and optimize your overall performance.

Practicing Golf with a Purpose

If you are like a lot of golfers out there, you spend an inordinate amount of time working on your game. Also, you practice a lot. You are not afraid of the driving range and putting green, even working to the point of having blisters on your hands. The problem is, though, no matter how much you practice, you just are not seeing the results you expect.

Don’t fret, this is true for a large number of golfers, and in the end it comes down to one thing, you must practice your golf game with a purpose. Computer Noise

Computer noise is normal … somewhat. Then there is the dreaded silence of death …

You see, there are several moving parts inside of your desktop computer. These parts are going to make noise, so that part is normal. Your computer has at least two fans, one on the CPU (central processing unit, or the brain of the computer) and the other on the power supply unit. An option third fan is sometimes located in the middle of the back of the case, directing airflow directly onto the CPU area, or venting the case itself. All of these fans will make noise.

In addition, your hard drive is comprised of small platters with magnetic read/write heads located in between the platters; a sort of sandwich if you will. Under normal operation, the hard drive is relatively silent, but as it gets older and it’s mechanisms get more worn, it will generate some sound when accessing the drive. This is a good indication that you need to start shopping for another hard drive … and don’t forget to regularly back-up the contents of the drive while you’re at it!

Two more things that generate noise on your computer are your CD-ROM (and DVD-ROM) drives and your floppy drive (if you still have one!). These devices spin your media at phenomenal rotation rates, and generate heat as well as noise. However, these devices have a noise that you should become fairly familiar with. Any noise that appears abnormal from these devices probably indicates a problem with the device. For example, a grinding noise in the floppy drive may indicate a bad floppy disk, or a bad drive. CD-ROM is the same way … a drive that has to be repeatedly opened and closed before it will access a CD could indicate a problem with the CD or the drive. In both cases, a careful examination of the media will help eliminate the media and point to the drive itself as the source of the problem. A good question to ask when encountering this type of issue is if the disk will work in another computer. If so, the problem is most likely your drive.

However, none of the things discussed so far describe the two most ominous sounds that can come from your computer: the whining grind of bearings going out, and complete silence. For the first issue, when the bearings in any of your fans start to go out, it starts to whine and grind. It will sound exactly like any other fan bearings that you might have heard go out … from an air conditioner unit to a box fan … the fan bearings all sound the same when they start to fail. At first this noise will be intermitten … but after a while it will become severe and nonstop. At this point it is imperative that you purchase a fan to replace the one that is going out. Why? Because what comes next can be very destructive …

Complete silence. No fan sounds. No grinding. Worst case scenario: it’s the CPU fan gone bad, and every second you use your computer you are burning up the CPU chip. Most computers today are designed to turn themselves off once the temperature on the CPU reaches a certain level … before damage takes place. However, this safety feature doesn’t always prevent damage from occurring. After you get the fan fixed, you can experience random shut-downs and system errors that are directly related to a previous overheating incident. So, it is best not to run your computer at all during times when overheating may be an issue.

So the bottom line? Get to know your computer! Some noise is normal … but excessive noise from any component on the computer usually indicates excessive wear and tear, not to mention possible emminent failure. So be alert and aware of what your computer sounds like now, … before it has any problems.

PPTP and HTTP Port Forwarding with Static NAT on a Cisco Router

Recently, a student at one of our seminars asked about port forwarding on a router. She wanted to allow PPTP clients to connect from the outside to a VPN server on the inside. In this article, I’ll explain how to do it along with a quick look at using static NAT to forward packets to a web server.

Port Forwarding on a Cisco Router

Sometimes we have internal resources that need to be Internet-accessible such as Web servers, mail servers, or VPN servers. Generally, I recommend isolating those resources in a DMZ to protect your office LAN from the bad guys, but regardless of how you choose to design it, the process involves forwarding desired packets from the router’s outside interface to an internal host. It’s really a fairly simple process. Here’s the configuration on a Cisco 2611 router:

In the above configuration, Ethernet 0/1 is connected to the public Internet with a static address of 12.1.2.3 and Ethernet 0/0 is connected to the inside network with a static address of 192.168.101.1. NAT outside is configured on E0/1 and NAT inside is configured on E0/0. Access-list 101 works in conjunction with the “ip nat inside source list 101 interface Ethernet0/1 overload” statement to permit all inside hosts to use E0/1 to connect to the Internet sharing whatever IP address is assigned to interface Ethernet E0/1.

The “overload” statement implements PAT (Port Address Translation) which makes that possible. (PAT allows multiple internal hosts to share single address on an external interface by appending different port numbers to each connection.)

The statement “ip nat inside source static tcp 192.168.101.2 1723 interface Ethernet0/1 1723″ takes incoming port 1723 (PPTP) requests on Ethernet0/1 and forwards them to the VPN server located at 192.168.101.2.

You could do something similar with a Web server by changing port 1723 to port 80 or port 443. Here’s what that would look like:

ip nat outside

interface Ethernet0/0
ip address 192.168.101.1 255.255.255.0
ip nat inside
!
ip nat inside source list 101 interface Ethernet0/1 overload
ip nat inside source static tcp 192.168.101.2 80 interface Ethernet0/1 80
!
access-list 101 permit ip any any

In this example, the web server is located at 192.168.101.2 and instead of forwarding PPTP (port 1723) traffic, we’re forwarding HTTP (port 80) traffic.

Obviously, you can configure your Cisco router in a similar manner to forward nearly any type of traffic from an outside interface to an internal host.
